Step 2
Email or Fax us a copy of your Sales Tax Permit (write down your email or phone number to identify):

Complete the form below and click on "Submit."
Steps 1 & Step 2 must be completed, or your request will NOT be approved.
Sample Order
If need sample order, place a regular order through non-wholesale account first. Then, send us an email with sample order number after placing your wholesale order in order to receive credit.
After steps 1, 2, and 3 are completed, we will send you a confirmation e-mail to inform you that we have activated your whole sale account with wholesale discount code. Once you apply this code during the checkout, discounted prices will be reflected.
